Morselized Bone Grafting in Osteolytic Lesions around the Stable Cementless Acetabular Cup

Abstract
For the treatment of osteolysis around cementless acetabular cup, we reviewed 26 patients who underwent curettage of granulation tissue, exchange of polyethylene insert and morselized bone grafting between from April 1996 to July 2000. The average followup period was 3.5 years. Harris Hip Score was adapted to judge clinical result. Radiologically, the size of osteolytic lesions in anteroposterior (AP) and lateral view was measured preoperatively and at the lastest followup, and a bone graft incorporation scale was used for evaluation of graft bone union. Clinically, average Harris Hip Score increased to 82 points postoperatively from 88 points preoperatively. The size of osteolytic lesions averaged 23.8 mm¡¿24.9 mm preoperatively and 11.5 mm¡¿8.2 mm at the last follow up in AP view, 11.1 mm¡¿11.6 mm preoperatively and 5.3 mm¡¿4.5 mm at the last follow up in lateral view. Bone graft incorporation scale was Grade I in 12 cases, Grade II in 14 cases at last follow up and new osteolytic lesion was not developed at the last follow up. For treatment of osteolysis around stable cementless acetabular cup, curettage of granulation tissue, exchange of polyethylene insert and morselized bone graft without change of acetabular cup can be effective treatment in prevention of progression of osteolytic lesion and preservation of bone stock for further revision total hip replacement.
